Probably there was a bit on one of the CNC turning centers that dulled -- and it didn't alarm until sometime after it built a few bad connectors -- and then they were missed by QC (just one thousandth of an inch in tolerance can cause the issue you described...)
I'd suggest you call tech support and just get them to send you an AWR. Tell 'em Patrick sent you.
