You won't hear from Jamie until next week at the earliest - he had a funeral to go to this weekend (Jamie had a bit of a rough week).
Regarding the holes - did you try all 4 adapters on the same axle or did one fit one axle and the other 3 didn't fit the next available axle? They are turned on a cnc lathe(each part is run exactly the same as the one before it), so I would suggest that the holes are all the same - perhaps a burr is present, or there is a slight tolerance variance on the axles or a combination of these issues?
I would shim the gap and run them - The idea is to see if the wide offset is going to cause any problems with the small axle and small bearings - the hole location, etc. are small details that can easily be worked out if they go into production - need to see howwell they work first, though. :)
