Upon further inspection, I decided to take the front plate off and check the bearing. Bearing is perfect, no play and smooth as glass, so I pulled the rotor out to check it. Well the kevlar or whatever, fiber wrap around the rotor is pretty badly frayed and separating from the rotor. I had a 60mm Medusa do this once, they replaced the rotor and good as new.
