40 series adaptors help please

Ok, i know i read up on this in a thread and now can't find it back, can someone help me figure out why i have binding now that i put the adaptors in? I'm sure it's because i have the spacers in wrong so it's stopping the bearings somehow. So if someone can give me a quick answer on how and where to put the spacers in? Location wise? Please???????
