My MMM needs to go in for repairs

Hi I have just competed the process on Castles website to return my MMM for repairs but not sure on one thing. It says to print the order confirmation. Now I am not sure what page that is. Is this the screen that I am meant to print and send with my MMM.

Last MMM I sent in my friend did it as he sent in his MMP as well so he did it for me. This time I want to do it my self but unsure on what to do. Any help would be appreciated.
