The removable rx wire would be a defect in the thickness of the pins or inserts in the wire plug, It was also one of the first run ESC's i think to where the problem may have been fixed already. I solved the wire problem by bending the 3 pins different directions so they hold pressure and then used a zip tie to prevent it from coming loose
Can you post up the setup you are seeing difficulty with? We just haven't seen an issue like this before. I'd like to have our engineers see if they can reproduce in the lab.

I tried different firmware and settings and it still did it. I already have my new replacement back so I can't look up anything anymore. I can send you my packing slip information if you want to try to find my old esc and play with it since you guys already have it there.

And back to the wires pulling out, you guys already fixed it, the new version has tabs to clip the wires in, my first one only had the friction of the pins fitting into the slots holding them in.

Here is the new one:
